How do I apply for a PGB?

Step 1.

Find out which care office is responsible for your application. In most cases, this is the municipality (see infographic).

  1. Apply for a PGB through the appropriate care officeโ€”this can usually be done online or by phone.

  2. The care office will review your personal situation and assess your care needs.

  3. They will determine what type of care you are entitled to.

  4. The health insurer, care office, or municipality will then decide whether a PGB will be granted and under which conditions.

Step 2.

During the application process, you will often be asked to explain why a PGB is suitable for your situation. Write your personal motivation in the form of a support plan.

It is recommended to request a specific number of sessions or hours per week rather than referring to an ISA Power package.
Weโ€™ve created a document that you can simply adapt to your own situation. This template will help you create a strong and convincing application (see downloads). Often the municipality will provide you with their own template for you to fill in.

Ask your general practitioner (GP) to provide a medical indication (referral) that supports the need for working with ISA Power.
For example:
"My patient (name) has been struggling with anorexia since mid-2024 and would benefit from individual support by an experienced professional, such as those at ISA Power. The unique support offered by ISA Power is not available to the client in any other setting."

Step 3

Once the care office agrees and issues an indication, you will receive a letter or message with the following information:

  • The duration of the indication

  • The approved hourly rate

  • The frequency of sessions per week/month, etc.

After this, the application is forwarded to the regional care office. You will then receive the necessary forms to officially allocate the budget.
You complete the care agreement (zorgovereenkomst) and submit it to the SVB (Social Insurance Bank). Once the SVB approves the agreement, you can start submitting the monthly invoices.
